

Service Coordination
Platte County Board of Services provides advocacy, coordination of services and linkages with community resources for County residents with developmental disabilities. This service is accredited by CARF.Service Coordination ensures that individuals receiving supports have access to and receive services that meet their needs. Service Coordinators also moniton the effectiveness of the services and supports being provided. Service Cooridination includes:
PLANNING SUPPORTS:
Planning Supports has two components: identifying the needs of the person being supported and creating an action plan that will support the person, his or her interests, talents, and gifts, how the person wants to learn, and what works best for the person. It also includes specific support strategies, timelines for completion, and identification of the person(s) responsible for each part of the plan.
LINKING RESOURCES:
The linking resources element matches the unique support needs of the individual and his/her family with resources in the community. Linking may involve researching existing resources, developing new resources, or making referrals to collaborating agencies.
QUALITY ENHANCEMENT:
Quality Enhancement encompasses the monitoring and assessment of the quality, timeliness, and effectiveness of services and supports received. This service component ensures the individuals with developmental disabilities receive the supports they need, when they need them.
At minimum, Service Coordinators have Bachelor's degrees in areas related to human services and a year of work in the field of developmental disabilties. They have all passed background checks through the Missouri Highway Patrol and the Missouri Family Care Registry. Additionally they hold certificates in CPR/First Aid, Medication Administration, and behavior intervention and have opportunities to enhance their knowledge through in-services, conferences, and other trainings.
Service Coordinators are typically available during normal business hours on weekdays.
Funding is primarily through Missouri HealthNet and contracts with the Missouri Division of Developmental Disabilities.
